Veracross Access Request Procedure
Veracross Access Request Procedure
- Click on the Access Request Form found at: http://apps.culver.org/accessrequest
- Enter the data below for each field:
- Date Requested: Today’s date
- Date access required: today’s date
- Action Requested: select Add Authorization in drop down menu
- Department: select your department in drop down menu
- Position: type your title
- Last Name: type your last name
- First Name: type your first name
- Veracross ID: type “Your Employee ID”
- Check the box next to each role you need to do your job.
- Scroll down to the bottom of the website and click the “Print Now” button
- Print form and provide to your Manager/Director for approval {written or PDF signatures are acceptable).
- If access to another department's module Is requested, obtain approval from that department's Director.
- Submit a helpdesk ticket under the Veracross category at https://heldesk.culver.org, Scan and attach all pages (including the IT use only page) of the completed form with all signature lines signed to the ticket.
